What to Expect
During your initial visit, you'll have the opportunity to meet your provider and discuss your concerns, goals, and expectations. This session serves as an opportunity for us to get to know you better and for you to learn more about our approach and how we can assist you.
Necessary Paperwork
To streamline your visit and make the most of our time together, we kindly ask that you complete several forms prior to your appointment. These forms are essential for understanding your needs and providing you with the best possible care. Please review the checklist below and ensure all items are completed for your first visit:
- 1. Create a login for the portal (email comes from '[email protected]')
- 2. Read and sign all consent forms (available on the portal)
- 3. Complete BVPS Intake Questions.
- 4. Complete forms coming from BVPS staff and [email protected]. Please check your spam if you do not see it in your inbox.
Time Zone Reminder
